Your Medicaid review process:
It may be time to renew!
You should have received a yellow notice (PDF) by mail or you may see a link highlighted in yellow in your NH EASY account. Check the due date. Not everyone will have the same due date.
Renew your Medicaid benefits within 60 days of the due date shown on your yellow letter or NH EASY account.
If you have not yet received your yellow notice and think you should have, please call 1-844-ASK-DHHS (1-844-275-3447).
Renew online, by mail, by phone, or in person.
- Online: Go to nheasy.nh.gov and make the updates using your NH EASY account.
- Mail: Send a copy of your completed Redetermination Form to the Central Scanning Unit, P.O. Box 181, Concord, NH 03302.
- Phone: 1-844-ASK-DHHS (1-844-275-3447)
- In person: Bring a copy of your Redetermination Form to any District Office that is convenient to you. You can find a list of District Offices at https://www.dhhs.nh.gov/about-dhhs/locations-facilities.
To renew your eligibility, you may need:
- Your Social Security number
- Proof of income (pay stubs)
- Your Medicaid Redetermination Form
- W-2 forms
